Complete The ONE Assessment online so we can understand your health history, current symptoms, lifestyle, concerns and goals.

Before your appointment, you will receive an order for The ONE Core Laboratory Assessment through Quest Diagnostics. Completing your laboratory testing in advance allows your available results to become part of your clinical review.

Laboratory testing is billed separately and is not included in the $475 assessment fee.

Before your visit, your completed health assessment and available laboratory findings are reviewed together to identify relevant patterns, questions and areas that may require further evaluation. This preparation allows your in-office assessment to begin with a more complete picture of you.

At your appointment, we will complete your InBody body composition analysis and vital signs, then bring your health history, laboratory findings, body composition and goals together to discuss appropriate next steps for your care.

Based on your assessment and clinical review, we will present and discuss a proposed care plan developed around your individual findings, needs and goals.

If you choose to move forward with your recommended treatment plan within 14 days of your assessment, a potential 50% credit ($237.50) may be applied toward an eligible treatment program, subject to our treatment credit policy.

When medically required before treatment, a telehealth evaluation with an appropriate U.S.-licensed medical provider will be completed before treatment begins. When clinically appropriate and available, treatment may begin the same day or be scheduled for a future visit.
